Plan your gift
You’ve worked hard; make your assets work toward your legacy. A planned gift to PCC continues your investment in our shared community.
There are two main ways to continue supporting PCC through your estate: naming it as a policy beneficiary or recognizing it in your will.
Policy Beneficiary: List The Primary Care Coalition of Montgomery County, Maryland, Inc. (52-1847976) as a beneficiary of your retirement or life insurance policies. These policies should allow you to allocate a percentage of the benefit value as well.
Will Bequest: Talk to your attorney about including PCC in your will. We have sample language below, but it is not intended to take the place of qualified legal advice.
"I hereby give to The Primary Care Coalition of Montgomery County, Maryland, Inc (52-1847976) the sum of $___________ [or "the rest, residue, and remainder of my estate"] [or "_____% of the rest, residue, and remainder of my estate"] for its unrestricted charitable use.”
Here are the organization details you will need to make PCC a beneficiary (or click here for a fillable and printable version):
Legal name: The Primary Care Coalition of Montgomery County, Maryland, Inc.
EIN: 52-1847976
Address: 8757 Georgia Ave, 10th Floor
Silver Spring, MD 20910
Please note: We are committed to honoring your gift as it was intended. If you would like to designate a purpose for your gift, we want to agree on appropriate long-term needs before you make any arrangements. Naming gifts or designations made to anything other than an existing program may be subject to Board approval before becoming effective. Please contact Stephanie Narayanan, Director of Development and External Relations before you arrange your gift: 301-628-3456 or stephanie_narayanan@primarycarecoalition.org. Thank you for helping us get it right!
